It's a privilege CD that you'd receive after buying 5000¥ worth of cheki at one of their December 2019 shows. Includes an unreleased track.
Upon receiving it you were supposed to sign it with your name (most likely to stop people from reselling). The CD itself doesn't have a cover, it's a plain CD-R that would later be signed by band members, next to the owner's name.
